Summary
BGPMA’s Benefit Program, including pre-tax deductions,
is designed to be over and above the standard. We respect each and
every employee as an important member of the BGPMA team, a value
that is reflected in our benefit package. The Benefit Program for
the 2003 calendar year is as follows:
HEALTH INSURANCE: BGPMA offers
a choice of either Harvard Pilgrim Health Plan or Neighborhood Health
Plan. This is a pre-tax deduction that you may arrange on an individual
or family plan. If you already have coverage under another medical
plan, you may elect to opt-out of medical coverage at BGPMA and
receive a supplement in your paycheck.
DENTAL AND VISION INSURANCE: One or both are available on an individual,
individual +1 or family basis.
LIFE INSURANCE & ACCIDENTAL DEATH & DISMEMBERMENT: This
is totally free of charge for you! The insurance plan has a value
equal to your annual salary, so as your salary increases so does
the value of your life insurance. You may also choose to purchase
additional insurance coverage for you and your dependents.
DISABILITY: A basic LTD benefit of 50% of your
salary, up to $7,500 per month, is offered at no charge to our employees
scheduled 24 hours or more. You may choose to purchase additional
long term disability benefit up to 60% of your salary. Should you
become sick or disabled you would receive either the 50% or 60%
of your gross monthly income, dependent on your plan of choice.
Additionally, you are eligible to purchase a short term disability
benefit.
401K PLAN : BGPMA contributes 50% up to your 6%
401K contribution toward your retirement into a 401K plan with investments
that you choose. You are eligible to receive this match after completing
one year of employment, age 21 and working at least 1,000 hours.
You are eligible to begin contributing pre-tax dollars on your own
during your first year of employment, contingent upon meeting the
eligibility requirements in the plan document.
BENEFIT TIME: This program allows you to take 18
days off during your first year of employment and up to 29 days
off after 15 years of employment. This Paid Time Off includes vacation,
holidays, sporadic illness and personal time. Plus, you accrue 2
days a year into an Extended Sick Leave Bank should you become ill
and need to be away from work for more than 3 days. You are also
allowed to cash in up to one week of time at the end of each year.
Benefit Time allows you to decide how you will use your time off.
Eligible staff receive paid time off for 6 major holidays.
SPENDING ACCOUNTS (Dependent Care and
Health Care): By opening a health care or dependent
care spending account, you may reimburse yourself for medical and/or
dependent care expenses on a tax favored basis, with money that
is not subject to federal, state or social security taxes.
CREDIT UNION: BGPMA works with the Crescent Credit
Union, offering an opportunity to have money deducted from your
paycheck on a bi-weekly basis to be deposited into a variety of
accounts, (checking, clubs and savings). Additionally, loans, including
home mortgages, are available. Credit Union membership offers a
convenient way to both save money and borrow money.
FITNESS FORUM: You are invited to purchase a membership
at Fitness Forum, a fully equipped and professionally staffed fitness
center located at 110 Liberty Street. Payments are deducted from
your paycheck.
DIRECT DEPOSIT: A program that allows you to have
your bi-weekly pay deposited directly to your bank account.
